Client Consent Agreement

Please read the following before signing this form.
By signing this agreement, I acknowledge that I have received satisfactory answers to any questions I had about my decision to get a tattoo from Suriya Tattoo Studio, and I agree to the following:

  1. Tattooing on fading area (3 Months guarantee for tattoo will not applicable)
  2. Tattooing on private parts (With knowledge of client gives permission to see and touch to get inked)
  3. Under the influence of alcohol and drugs (3 Months guarantee for tattoo will not applicable)
  4. Client gives permission to take photographs of the tattoo and post it on social media.
  5. Hygiene and Safety: I acknowledge strict hygiene and safety protocols, including sterile equipment and single-use needles.
  6. Pain and Discomfort: I understand the tattooing process may cause discomfort or pain.
  7. Aftercare Instructions: I agree to follow aftercare instructions to ensure proper healing.
  8. Touch-up sessions: I agree to attend recommended follow-up appointments (within 3 months).
  9. Release of liability: I release Suriya Tattoo Studio from liability except in cases of negligence or misconduct.
